To determine the frequency of pulp alterations in patients who are going to begin orthodontic treatment. Methods: Cross-sectional study. Included patients ≥ 18 years, with periapical radiographic series and and they were to start orthodontic first. Patients <18 years should have complete root formation. We excluded patients with pacemakers or previous orthodontic treatment. Clinical examination pulp and periapical state with vitality and sensitivity tests. The absolute and relative frequencies for pulp and peri-radicular pulp diagnostics, were described and stratified by sex, age, arch and area. Results: We analyzed 37 patients (19 women, 18 men) with 981 teeth. 48.6% had pulp or periapical alterations. Depulped teeth were found more often in women (72.2%), with 41 years old in average and patients who had restorations of 3 or more surfaces. Of the 39 affected teeth, 20.5% had periapical alteration. The most common teeth with pulp and periapical alteration were the first left upper molar. Of all 34 teeth with root canal treatment, 73.5% of the root canals were not in a good condition of condensation or obturation. 74.3% of the altered teeth were in the upper arch and 64.1% in the posterior area. Conclusions: The frequency of pulpal or periapical alteration is very common in patients that are going to begin a orthodontic treatment, so it is required by the endodontist prior assessment before starting treatment.
