Introduction: the hypertensive patient who has good adherence to both pharmacological and non-pharmacological treatment, has a better chance of taking his disease without complications, of better control of his blood pressure and therefore to better benefit from changes in his style of life. Objective: to identify the factors related to adherence to antihypertensive treatment. Methodology: a narrative review of articles from the database of the Cooperative University of Colombia and open databases, published since 2005 with a quantitative methodology in Spanish, Portuguese and English, was carried out. data analysis and obtaining results. Results: adherence to treatment was reported above 50% in most of the selected investigations; Regarding gender, women were more adherent than men (56.3% versus 48.6%); It was found that the type of medication that predominated in this study was monotherapy, ranging from 13.2% to 83.41% of its participants; between 50.5% to 80% of the people interviewed received family support; patients with good knowledge about the disease were seven times more likely to have good adherence to antihypertensive drugs than those with little knowledge (AOR 7.4 [95% CI: 4.177- 13.121], p <0.001). Conclusions: according to the results, the factors that favor adherence to antihypertensive treatment are mentioned below: female patients, family support, good knowledge about their disease, longer treatment time and those who had monotherapy treatment Key words: compliance and adherence to treatment, hypertension, health education, knowledge of medication by the patient.
