Introduction: The Objective Structured Clinical Evaluation (OSCE) is one of the most valid methods for evaluating medical competencies. Objective: To evaluate the impact of the OSCE on the medical competences of interns of the Faculty of Medicine of the Universidad Cooperativa de Colombia. Methods: 289 medical internship of eight cohorts of the faculty were studied. An OSCE was designed and validated for emergencies, pediatrics, internal medicine, gynecology, surgery and orthopedics, which were applied at two different times, and compared with the traditional evaluation. The impact of the OSCE was determined with the statistical method of Differences in Differences. Regression models were made to identify the factors associated with the performance of students. The analyzes were performed in SPSS 25.0® with significance of 0.05. Results: 63% were women, 77.5% of middle class, 91% of single marital status and only one third of Medellin. Academic performance was better with the OSCE compared to the traditional evaluation, especially in emergencies where it was 0.45 higher, pediatrics with 0.90 and gynecology with 1.0 (on a scale of 0.0 to 5.0). In internal medicine, no differences were found between the two evaluation modalities. The differences in emergencies presented association with the cohort and the academic performance (academic notes), in pediatrics and internal medicine they presented association with the notes in the internship, in gynecology with the notes in the internship and the cohort. Conclusion: The OSCE had a positive impact on the academic performance of medical internship, with some differential results according to cohort and global academic average. This constitutes an important baseline for subsequent research in Colombia, where training and evaluation by competences is scarce.
