This work arises from the importance of self-evaluation, monitoring and prioritization of healthcare standards in the generation of an improvement plan on the path that this institution has in search of Accreditation, using as an evaluation and monitoring tool the instrument called Tracer Patient in the institution of high complexity for the benefit of meeting the proposed goal to obtain the Accreditation, to achieve it would be one of the first institutions accredited in the Coffee Axis, joining the few institutions in the country that have these maximum quality standards and aimed at excellence Through the adaptation of an instrument for Tracer Patient based on the Accreditation Manual according to version 3.1 published in 2018 (see annex 3) which is applied to 4 patients randomly that meet certain inclusion criteria with management in the institution, subsequently an analysis of the failures found in each patient that coincide in general is performed and on this an improvement plan is made, in which details each fault with their respective recommendations. Finally, an instrument for an objective Tracer Patient with an analysis of quality care is delivered to the institution, through its application where the critical points to work are evidenced; a detailed action plan that provides information on how critical points will be corrected and the institution's obligation to act on the failures that are delivered
