The relationship between osteonecrosis associated with the use of bisphosphonates is a rare pathology, but with serious complications due to the lack of knowledge of physicians and dentists about drug-induced reactions. This produces a deterioration in the quality of life of the patients and a negative impact on health indicators and costs. Osteonecrosis (bone death), also known as avascular or aseptic necrosis, is a consequence of the final process of severe reduction or absence of irrigation of the bone, resulting in bone and surrounding tissue death. There are multiple causes, such as antiresorptive medications (bisphosphonates), high steroid therapy, cancer, radio and chemotherapy, autoimmune, genetic, osteolytic and osteoblastic diseases, alcoholism, traumatic injuries, among others. The American Association of Maxillofacial Surgery (AAOMS), in 2007, defines osteonecrosis associated with bisphosphonates (ONAB) as bone exposure for more than 8 weeks in patients with treatment of bisphosphonates (Bf) and free of radiotherapy in the maxillofacial region. This document presents a literature review about the progress of research since the relationship between bisphosphonates and osteonecrosis of the jaws was observed in 2003 until the actuality. Thus, the still existing gap of studies that guide the development of clear protocols for the promotion of health, prevention of complications, early diagnosis and effective treatment was verified.
