    Variante folicular de carcinoma papilar de tiroides con metástasis a tejidos blandos. Reporte de caso

    Introduction: Papillary thyroid carcinoma has a variable biological behavior. Usually it can cause lymph node, lung or bone metastasis; however, soft tissue metastasis cases are very rare. Presentation of the case: A 78-year-old man from an urban area attended the outpatient clinic of a tertiary care hospital in Pasto, Colombia, due to a painful mass in his right thigh. The patient had a history of papillary thyroid carcinoma that had been treated 30 years before his consultation by means of total thyroidectomy plus radical neck dissection. Then, a 6x4 cm mass was observed in a MRI, thus the tumor was fully resected in a surgical procedure without any complications. The diagnosis of follicular variant of papillary thyroid carcinoma with soft-tissue metastasis was confirmed by the histopathological report,. At present, the patient's condition is good, without metastatic recurrence and he is under constant monitoring to detect possible new distant metastases. Conclusion: Although soft tissue metastasis cases from papillary thyroid carcinoma are rare, this condition should always be suspected in patients with a history of thyroid carcinoma regardless of the time of the follow-up elapsed.

    Gist gigante de alto riesgo: Reporte de caso y revisión de la literatura

    Gastrointestinal stromal tumors (GIST) are the most common mesenchymal neoplasms of the digestive tract. Risk staging criteria such as angioinvasion, size, infiltration, and mitotic index allow for adequate diagnosis and treatment. A 67-year-old male patient is presented, who consulted for hematemesis and melena. Endoscopically, a 20x10x6 cm diameter mass was observed in the stomach, which was taken for a biopsy for study. On histopathological examination, a GIST with the microscopic invasion of the lamina propia and angioinvasion was diagnosed. These two histopathological components are scarcely taken into account to classify GIST; however, they are very important in this context, changing the prognosis of the disease and requiring adjuvant therapy with imatinib. The outcome of the patient was favorable due to the established therapeutic conduct. The biological behavior of GIST varies depending on the clinical and histopathological characteristics. This case highlights the high- risk criteria for GIST and the need to modify the treatment when present.

    Tumor de células granulares en región axilar: a propósito de un caso y revisión de la literatura

    Abstract Granular cell tumor is a rare neoplasm with benign behavior. It usually occurs in the fourth to sixth decade of life as a solitary, slow growing lesion with a good prognosis. Its histogenesis is probably of neural origin, being positive for S-100 and Neuron-Specific Enolase. We demonstrate an unusual location in the axillary region, the obstacles to reaching the diagnosis since it can be confused with other malignancies, and the essential elements for clinically suspecting benign lesions of this type. Resumen El tumor de células granulares es una neoplasia infrecuente, de comportamiento biológico benigno. Por lo general, se presenta entre la cuarta y sexta década de vida como una lesión solitaria, de crecimiento lento y buen pronóstico. Su histogénesis es probablemente de origen neural siendo positivo para S-100 y Enolasa Neuronal Especifica. Se muestra un caso con una localización inusual en la región axilar, las dificultades para alcanzar el diagnóstico puesto que puede confundirse con otras neoplasias, y los elementos clínicos esenciales de este tipo de tumor.

    Sociodemographic and histopathological characteristics of patients with gastric cancer in a high-risk population in Colombia

    Objective: To describe the sociodemographic and histopathological characteristics of patients diagnosed with gastric adenocarcinoma at the Clínica Oncológica Aurora between 2014 and 2017 in the city of Pasto, Colombia. Methodology: A descriptive study was carried out in a cohort of 54 patients diagnosed with gastric cancer who underwent gastrectomy between 2014 and 2017. Sociodemographic information was taken from the medical records of the patients. The Sydney system and Lauren classification were used to determine histopathological characteristics. Results: Most tumors were found in men older than 50 years (with a male:female ratio of 2.6:1). The predominant location was the antropyloric region. The most frequent tumor histotype was intestinal (80 %). The prevalence of Helicobacter pylori in patients was 24.07 % and it was most commonly found in the intestinal histotype. Intestinal metaplasia was the most prevalent premalignant lesion in all tumor histotypes. Conclusions: The present study found that the age of 50 and the male sex are conditions associated with gastric cancer. This finding has already been demonstrated in previous studies. Progress in improving public health conditions, controlling H. pylori infection, and early screening of premalignant lesions is imperative, as they are determining factors of carcinogenesis in patients with non-cardia carcinomas and intestinal histotypes located in the anthrocorporal region.     Criterios para el uso e investigación de medicamentos off-label frente al covid-19 en Latinoamérica

    The COVID-19 pandemic represents one of the most important challenges in public health’s history. Severity of the clinical picture and lack of supporting scientific evidence have led to compassionate use of off-label medications. Indiscriminate use of these medications, however, can be harmful for patients. The Latin American medical and scientific community is called upon to evaluate research hypotheses of treatments that could mitigate the impact of the pandemic on our populations.Taking the use of ivermectin as an example, various criteria such as biological plausibility, methodological and statistically acceptable evidence, a favorable clinical response, a safe pharmacological profile, feasibility, and altruistic interest are proposed to support the use of off-label treatments during the pandemic. These criteria would allow a more rational use of medications and prioritize research on potentially effective therapeutic options.

    Mathematical Model of HIV/AIDS Considering Sexual Preferences Under Antiretroviral Therapy, a Case Study in San Juan de Pasto, Colombia.

    While several studies on human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) in the homosexual and heterosexual population have demonstrated substantial advantages in controlling HIV transmission in these groups, the overall benefits of the models with a bisexual population and initiation of antiretroviral therapy have not had enough attention in dynamic modeling. Thus, we used a mathematical model based on studying the impacts of bisexual behavior in a global community developed in the PhD thesis work of Espitia (2021). The model is governed by a nonlinear ordinary differential equation system, the parameters of which are calibrated with data from the cumulative cases of HIV infection and AIDSreported in San Juan de Pasto in 2019. Ourmodel estimations showwhich parameters are the most influential and how to modulate them to decrease the HIV infection.

    Practical Guide Model for Developing Morphology Laboratory Practices: This material aims to facilitate the learning of the histology of the body systems in the course of the Linfocardiovascular System of the third semester of the Medicine Program of the Cooperative University of Colombia. The guide maintains a logical sequence for the observation of the structures of the lymphocardiovascular system, initially describes the histological components of each of the blood vessels: Arteries, veins and capillaries, as well as details the histological components of the heart: ventricles, atria and mitral, tricuspid, aortic and pulmonary heart valves. A technical description of the topic is provided at the beginning of the Guide to facilitate the description and visualization of microscopic structures and components of vascular tissue. This guide can also be used in histology laboratory practices in programs such as Dentistry, Nursing and Veterinary Medicine
